Company History

Community Health Connections was founded in 2002 and has amassed decades of experience as a Federally Qualified Health Center (FQHC). Over the years, it has expanded its reach to five locations across Fitchburg, Gardner, and Leominster, Massachusetts. Community Health Connections is dedicated to serving 36 communities within North Central Massachusetts, consistently striving to provide high-quality health care to all individuals regardless of their income or health insurance status.

Services Offered

Community Health Connections offers a comprehensive range of health services. These include primary family medical care, preventative and restorative dental care, and oral surgery. The organization also provides behavioral health services, substance use disorder treatment, and integrated specialty services such as optometry, acupuncture, nutrition consultations, and podiatry. Additionally, Community Health Connections offers walk-in urgent care services at multiple locations and provides telehealth services for various medical and behavioral health needs.

Patient Care and Access

In 2023, Community Health Connections served 32,375 patients and provided 129,194 in-person and telehealth visits. All of the organization's health centers are accessible via the Montachusett Regional Transit Authority (MART) bus line, ensuring ease of access for patients. The organization supports health care access regardless of income, and offers a sliding fee scale based on income and family size, along with assistance in navigating and obtaining affordable health insurance options.

Community Engagement and Programs

Community Health Connections is deeply involved in community engagement initiatives. It partners with Fitchburg State University to deliver healthcare services to students and collaborates with New York University’s Langone Health System for a Dental Residency Program. The organization also runs the Caring For Kids program, providing preventive dental care at local schools. Beyond education, it supports migratory and seasonal agricultural workers through the Connecticut River Valley Farmworker Health Program and offers dedicated services for the homeless and those in unstable housing situations.
