It offers a three-click process for addressing care opportunities, including closing HEDIS and HCC measures. CareEmpower® consolidates value-based care workflows, task lists, reporting, and payments into a single environment.

Community Partnerships

The Equality Health Foundation collaborates with local nonprofits, health plans, providers, and other community partners to advance equal health for all. Through these partnerships, the foundation extends its impact into local communities by promoting health education and eliminating healthcare inequalities. Equality Health also works with 25 health plans on value-based arrangements.
