These services include medical and dental care for all ages, behavioral health, family planning, and chiropractic services. Patients also have access to a pharmacy that provides financial assistance with medications and free home delivery of prescriptions. Additional free support services include care coordination, health insurance enrollment, and medical translation.

Patient Demographics

The Chautauqua Center serves a diverse patient population, with 970 patients aged 60+, and 32% of patients being children. Furthermore, 72% of patients are below the poverty line, and 16% of patients are Spanish speaking. This demographic information underscores the clinic's commitment to serving vulnerable and underserved communities.

The Chautauqua Center has earned significant recognition for its contributions to healthcare. In 2020, it received the Economic Development Award from the Chautauqua County Chamber of Commerce. It has also been recognized by the Health Resources & Service Administration (HRSA) in three categories of achievement: Health Disparities Reducer, Access Enhancer, and Advancing Health Information Technology (HIT) for Quality. Additionally, it received the designation of a Patient Centered Medical Home (PCMH) in 2017.
