Trenton Health Team (THT) was founded in 2006 to improve access to quality healthcare for residents of Trenton, NJ. The organization is a multi-sector partnership that focuses on advancing the health and well-being of the greater Trenton community. THT employs a data-driven approach to healthcare, continuously analyzing and sharing data to enhance community well-being.
Services
Trenton Health Team (THT) provides a wide range of services aimed at improving health outcomes for Trenton residents. Key services include free health screenings provided by Community Health Workers, care management through the Care Management Team (CMT), and support for residents with complex health and social conditions via the Community Care Team. THT also focuses on addressing Social Determinants of Health (SDOH) and helping patients access services related to food, clothing, shelter, and finances.
Initiatives
Trenton Health Team (THT) runs numerous initiatives to promote health and well-being in the community. These initiatives include Cancer Education and Early Detection, Capital City Diabetes Collaborative, Community Care, Community Health Improvement Plan, Faith in Prevention, Family Connects, Get Covered, Health Screenings, Healthy Housing, NJ Health Indicators, NJ Regional Health Hubs, Trenton Health Information Exchange (HIE), and the Transforming Communities Initiative.
Community Advisory Board
The Community Advisory Board of Trenton Health Team (THT) represents over 100 organizations and 500 individuals. The board regularly convenes to share information, brainstorm solutions, and co-design interventions to enhance health and well-being. Members include organizations such as Capital Health, Central Jersey Family Health Consortium, and Princeton University. The board also features workgroups focusing on specific issues like access to healthy food and maternal health, as well as affinity groups that foster collaborations among colleagues such as community health workers and communications directors.
